Independent investigation into the healthcare and treatment of Mr A. On 1 May 2013, Mr A stabbed and killed his wife, then committed suicide.
At the time of the offence, Mr A was receiving care from Northumberland, Tyne and Wear NHS Foundation Trust, provider of mental health services.
Following the completion of legal proceedings, NHS England commissioned Verita, an organisation which specialises in public sector investigations, to conduct an independent investigation into the healthcare and treatment provided to Mr A, to see if there were any lessons to be learned.
The panel’s findings will be used by local clinical commissioning groups, who have responsibility for purchasing mental health services for local people, in order to inform future commissioning criteria and decisions.
Independent investigation into the care and treatment of Mr B
This is the independent investigation into the care and treatment of Mr B, who was found guilty of murder and sentenced to life imprisonment after killing Mr X.
At the time of the murder, Mr B was receiving mental health services provided by Northumberland, Tyne and Wear NHS Foundation Trust.
